Your Social Insurance Number (SIN), date of birth and full legal name, current address, details of any other government dental coverage, and your latest CRA Notice of Assessment.
Do I need to be a Canadian citizen?
No. You only need to be a Canadian resident for tax purposes, have filed your most recent tax return, and meet the eligibility criteria.
Can my family members apply too?
Yes. Each eligible family member needs to submit their own application.
How long does it take to be approved?
It typically takes a few weeks. You’ll receive confirmation online via your CRA My Account or by mail.
Do I have to reapply each year?
No. As long as you continue to qualify, your CDCP coverage will automatically renew based on your latest tax return.
